Client information & consent to treatment

This information applies to all Work With Care clients and is intended to clearly inform you in advance of treatment, your rights and our practices. You do not have to fill in or sign anything.

1. Who are we?

Work With Care is a physical therapy practice utilizing a network of independent, licensed health care providers. When you make an appointment, the treatment agreement is made directly with the respective practitioner providing the care. Your practitioner is the direct point of contact for your medical records and the implementation of treatment.

2. Treatment and agreement (WGBO).

When you make an appointment and participate in treatment, a treatment agreement as referred to in the Medical Treatment Agreement Act (WGBO) automatically arises.

Prior to and during treatment you will be informed about:

  • The purpose and content of the treatment;
  • possible risks and alternatives;
  • the expected duration;
  • the cost.

By participating in the treatment, you consent to its implementation.

3. Your rights and obligations

Your rights

Your rights include:

  • clear information about your treatment;
  • access to and a copy of your file;
  • correction or (partial) destruction of your file;
  • privacy and confidential handling of your information.

Your duties

You are expected to:

  • provides accurate and complete information relevant to treatment;
  • communicate changes in your health in a timely manner;
  • provides feedback on the course of treatment.
4. Privacy and personal data (AVG)

To provide care, we process personal and medical data. This is done carefully and according to the General Data Protection Regulation (AVG).

  • Your records will be recorded in a secure electronic health record (EHR), such as SmartFile.
  • Medical records are kept for a minimum of 20 years (WGBO).
  • Administrative and billing records are kept for 7 years (tax laws).


Data will be shared with third parties (such as your primary care physician) only when necessary for treatment or when you have given permission.

5. Communication

We may, if desired, contact you via email, phone, WhatsApp or SMS. Digital communication is applied securely as much as possible.

6. Direct Access Physical Therapy (DTF).

If you register yourself without a physician’s referral, a screening will first be conducted to assess whether physical therapy treatment is warranted. If necessary, we advise you to contact your primary care physician.

7. Appointments, cancellations and rates
  • Appointments can be cancelled or rescheduled free of charge up to 24 hours prior to the appointment.
  • Late cancellation or no-show at the appointment may be charged at the rate of the reserved time.
  • The applicable rates can be viewed in advance on our website.


You are responsible for checking for any reimbursements from your health insurance company.

8. Liability
  • We naturally do everything possible to provide you with the very best care and guidance. In the unlikely event that something does go wrong, the liability of the therapist and Work With Care for direct damage is always limited. This means that the compensation never exceeds the amount that the professional or company liability insurance pays out in that specific case, plus the excess.
  • Liability for indirect or consequential damages is hereby excluded.
  • Finally, a friendly reminder: please pay close attention to your personal belongings (such as your coat, purse or phone) during your visit. We unfortunately cannot be held responsible for loss, damage or theft in or around the practice location.
9. Complaints procedure (Wkkgz).

Work With Care complies with the Care Quality, Complaints and Disputes Act (Wkkgz) and is affiliated with a recognized complaint and dispute resolution system.

If you have a complaint, we ask you to discuss it with us first. If you cannot resolve the matter with us, you can contact the independent arbitration board to which we are affiliated.
